This is a dry, heavily statistical look at foreign aid, from developed countries to the developing world. The causes were invariably worthy, like fighting malaria or AIDS.

Debt restructuring is also studied. Along with the corresponding topic of debt relief, where debt would be written off by the creditors (nations or banks). Here, there is a recap of the Baker plan (1985), the Lawson plan (1987), and the Brady plan (1989). Plus the informal Paris Club. More recent efforts include a call for 100% debt cancellation, as suggested by the US and British finance ministers in 2005. Unlike trendy or populist efforts that support this total debt cancellation for some nations, the book also explains the counter arguments. That this disfavours other nations with debt, that won't get the cancellations. Or that the "lucky" nations who do, will still need to borrow for future programs. And that the cancellations offer little incentive for them to improve their governance (like reducing corruption). In effect, it penalises those countries who have good policies and used aid and loans effectively.

In the Turkish elections of December 1995, the Islamic Welfare Party became the biggest Party in parliament� and for the first time in history, an Islamic party had come to power by means of free elections.
The rise to power of the Turkish Islamists is a result of several decades of revivalism. In this process the veil has been a prominent symbol of the new religious puritanism, causing resentment among those who regard the bare-headed woman as the symbol of progress and emancipation. In the light of a century-long conflict between secularism and popular Islam, the present study describes the conflict over the veil as it became a burning issue in the decade following the military intervention of 1980 and remains to this day a matter of controversy.
While focusing on the issue of veiling, the author also considers the wider picture of tension between official secularism and popular Islam in present-day Turkey. Although� this tension is not discounted, the author argues that the fact that the Islamic movement is on the rise does not mean that it threatens the very foundations of modern Turkish society. Whereas the controversies of the nineteenth century could be described as a 'clash of civilizations' (between Islam and the West), those of today have shrunk into conflicts over certain cultural symbols that are part of the same globally-expanding technological civilization.

A very well-written history of the conflict over veiling in Turkey in the 80s and 90s. The author is a professor of sociology in Ankara, and unlike many of her colleagues can actually write clearly. I love anthro and soc books but want them to be written to the standard of good novels and not dry, otherwise I can't read them. I picked this book up in Turkey mainly because I opened it and found it was well-written, which makes anything it talks about interesting. There is a history of veiling before the 1980s in Turkey, then more detail about the issue in the 80s and 90s, with 3 interesting case studies/interviews with women who choose to veil or not to veil. A good book to read about Turkish culture.

The choice of this topic for an issue of the PERSPECTIVES IN CRITICAL CARE INFECTIOUS DISEASES Series is therefore quite appropriate.
Despite its importance, many areas of the management of ventilator-associated pneumonia remain controversial. We therefore are pleased to include contributions from experts and investigators offering different perspectives on some of these controversial areas.

Ventilator-Associated Pneumonia (Perspectives on Critical Care Infectious Diseases) Review

Most of the edited, multiauthor texts available in the medical marketplace avoid repetitive coverage of specific topics by design, if not always in practice. For instance, a typical book on ventilator-associated pneumonia (VAP) would include one chapter on epidemiology, one on diagnosis, one on treatment, and so on. The series Perspectives on Critical Care Infectious Diseases, of which the present volume represents the fourth contribution, is quite different in that any given subject is discussed by more than one author or group of authors. As stated in the preface by the series editor, Jordi Rello, MD, this was a calculated plan meant to present"...the different management perspectives for these controversial and evolving topics." Dr. Rello goes on t specify that two guest editors are chosen for each volume, one from Europe and one from North America, thus further guaranteeing a more international perspective. After reading Ventilator-Associated Pneumonia, it is clear that such an approach has many advantages for the sophisticated reader but presents a significant challenge to the novice.
The book begins with two discussions of diagnostic strategies for VAP, as presented by Jean-Yves Fagon and Jean Chastre (presumably, the European experience) and Michael S. Niederman (presumably, the North American counterpart). The editors clearly chose their authors wisely, since one could hardly imagine two more opposite viewpoints. the first advocates invasive, quantitative culture techniques while the latter maintains that a "clinical and empiric approach" makes better sense. Both chapters marshal considerable supporting evidence from the literature, but neither can cite definitive randomized controlled trials providing unequivocal evidence of the superiority of either approach. The reader is still left with the essential controversy unresolved. While the empiric view may be easier and more convenient, a diagnosis that relies on "a new or progressive infiltrate with at least 2 of the following 3 criteria: fever, purulent sputum, or leukocytosis" can hardly be specific when other common intensive care syndromes can and often do, duplicate these findings. On the other hand, quantitative culture techniques may lack sensitivity, particularly in the case of early infection and may be prone to false-negative results, especially when performed after antibiotic treatment already has been started.
The remainder of the book seems far less controversial. The prevention of VAP is covered by two different authors, the pathogenesis of VAP (including the putative roles of the oropharynx and digestive tract) is covered in one chapter, and the treatment of VAP in Europe and North America is discussed by representatives from those respective medical communities. The text concludes with two views on the emergence and control of resistant organisms, and with a brief chapter further discussing some of the controversies existing in the VAP literature (as exemplified in the previous chapers). This last chapter provides a degree of summation and resolution for the opposing opinions presented earlier in the book, but it could have been more comprehensive in that regard. Also, this particular chapter contained numerous grammatical and typographic errors that interfered with readability. There is much repetition present throughout the text, mainly among the chapters on treatment and antibiotic resistance, but this is certainly unavoidable given the central premise of a book that seeks to present differing opinions on identical topics.
Ventilator-Associated Pneumonia serves the needs of a more restricted audience compared to other texts. Since it presents varyng points of view on many topices, it is not particularly suitable for medical students, residents, or fellows who are new to critical care medicine and who will have difficulty coming away with a unified "game plan" for VAP management. Rather, this book is a usefull tool for the experienced critical care, pulmonary, or infectious disease clinician or researcher, who can interpret the material presented in light of his/her own experience and knowledge, and perhaps alter an already mature practice style accordingly.
